Selection of Binocular Acuity Thresholds in the SiVIEW Exam Application

1. Introduction

The SiVIEW Exam application allows you to conduct in-depth vision exams using different binocular acuity thresholds. This guide provides detailed explanation on:

The goal is to help users choose the most appropriate binocular acuity threshold based on the patient’s characteristics

2. Presentation of Binocular Acuity Thresholds

The SiVIEW Exam application offers two binocular acuity thresholds for in-depth vision assessments:

Low threshold : Recommended for certain patients based on their profile. It allows for a precise evalutation without excessive strain on visual acuity.
High threshold : A more demanding threshold used to assess visual acuity above, the average level.

3. Selection Criteria for Acuity Threshold

3.1 Automatic Selection by the Application

The application automatically detects if the patient is 71 years or older and displays a warning message recommending a Low Threshold

Message displayed at the start of the exam :

“Given the patient’s age, we recommend a deep exam with a binocular acuity ceiling of 12/10”

3.2 Other Selection Criteria

In some cases, no warning is displayed, but it is strongly recommended to select the Low Threshold if the patient meets any of the following criteria :

A. Ocular Pathologies

  • Any ocular condition recorded in the patient’s Case History (CH)
  • Amblyopia, a reduction in visual acuity in one eye despite proper optical correction.

B. General Pathologies

  • Certain systemic diseases affecting vision (e.g, diabetes, neurological disorders, macular degeneration)

C. High Optical Correction

A Low Threshold is recommended if optical correction exceeds :

  • Myopia 6D
  • Hyperopia 4D
  • Astigmatism (Cylinder) 2D
  • Sphere difference between OD and OS 1,5D

4. Consesquences of Choosing the Wrong Threshold

4.1 If a threshold that is too high is used (High Threshold instead of Low Threshold)

  • The exam may overestimate the patient’s visual abilities.
  • Increased visual fatigue, especially for older patients or those with conditions
  • The test results may be less reliable, particularly for patients with strong optical corrections.

4.2 If a threshold that is too low is used (Low Threshold instead of High Threshold)

  • The exam may fail to detect superior visual acuity in some patients.
  • Patients with exceptional vision may be under-evaluated.

5. Recommendations and Best Practices

5.1 verify Patient Information

Before starting the exam, ensure that the patient’s details are correctly entered in the application :

  • Correct age
  • Recorded pathologies in the Case History (CH)
  • Accurate optical correction

5.2 Follow the Recommended Thresholds

  • By default, use the High Threshold unless the Low Threshold criteria is apply
  • Do not ignore application warnings

5.3 What if I'm unsure about which threshold to choose

If you’re unsure between Low Threshold and High Threshold, favor Low Threshold if the patient meets any of the selection criteria (age, pathologies, high optical correction).
